NHTD Waiver Program
The New York State Department of Health NHTD Waiver Program is designed to assist individuals with ongoing health issues so that they can remain living in their own home.
TBI Waiver Program
Designed to provide supports and services to individuals recovering from a traumatic brain injury.
The Service Coordinator assists individuals to become a waiver participant and coordinates and monitors the provision of all services in the Service Plan. Services may include Medicaid State Plan services, non-Medicaid federal, state and locally funded services, as well as educational, vocational, social, and medical services. The goal is to increase the participant’s independence, productivity and integration into the community while maintaining the health and welfare of the individual.

Skills Training)

ILST (Independent Living Skills Training)
assists with banking, meal prep,
organization, learning medicaiton
management and so much more.
CIC is an individualized service designed to assist the waiver participant to more effectively manage the emotional difficulties associated with adjusting to and living in the community. It is a counseling service provided to a participant coping with altered abilities and skills, the need to revise long term expectations, and changed roles in relation to significant others. We provide this service in the participant’s home. There are times when it is appropriate to include the participant in a family counseling or group counseling setting.
CTS-Community Transitional Support
Available to those on the TBI/NHTD Waivers who are moving home from a nursing home or rehab.
We can provide the funds to get clients into their own home. If you or your loved one needs financial assistance to enable moving companies to get your things home, we are here to help! Contact us today to get moving!
E-Mods ( Environmental Modifications) are those changes to your home that are necessary for you to be as independent as possible and remain in your home. Such as: Wheelchair Ramps, Widened Doorways, or Modified Bathrooms.
Our experienced and licensed contractors are paid through the NHTD/TBI waiver, at no expense to you, so that you can stay living in your home.
